Therefore, a 3D model was created, and the parameters of line spacing, nipple position, breast projection, surface area, and volume were evaluated. Furthermore, consider the difference in chest circumferences and breast volume.
Results
In comparison with the simulation results, the actual results had a mean increase in the nipple to the inframammary fold (N-IMF) of 0.3 cm (P < 0.05) and a mean increase in basal breast width (BW) of 0.3 cm (P < 0.001), a difference that was not statistically significant in patients with larger breast volumes. There was a significant difference in the mean upper and lower breast volume distribution between simulated and actual breasts (upper pole 52.9% vs. 49.2%, P < 0.05, and lower pole 47.1% vs. 50.8%, P < 0.001). However, it was not statistically significant in patients with larger chest circumferences.
Conclusions
Our study shows that 3D simulation has uncertainties related to the patient's chest circumference and breast volume. Therefore, these two critical factors must be considered when using simulation assessment in preoperative planning.
Level of evidence iii
This journal requires that authors assign a level of evidence to each article. The standard dimensions on which most algorithms are based are width, height and the projection of the silicone gel-filled implants. The ARC is introduced as a new dimension instead of the currently used anatomical silicone gel-filled implants parameters. The algorithm presented in this article is based on the amount of breast parenchyma (P) and the ARC of an anatomical silicone gel-filled implant to obtain an exact positioning of the incision at the new inframammary fold. A patient case is presented to demonstrate the use of the algorithm. To date, this technique has been used in over 1200 patients with reproducible and consistent results. However, multiple factors may cause a miscommunication between the surgeon and patient when brassiere cup size is used as a measure of results. The aim of this study was to determine the degree of agreement between disclosed and estimated brassiere cup size and also interrater agreement.
Methods
Three-dimensional (3D) scans of 32 subjects were evaluated by 10 plastic surgeons estimating cup size using the American brassiere system. The surgeons were blinded to all parameters, including the 3D surface software-derived volume measures of the Vectra scan. The 3D scans of the anterior torsos were viewed. The plastic surgeons' estimations were compared with the cup sizes stated by the subjects (disclosed cup size), using simple and weighted Kappa statistics.
Results
Agreement between the estimated and disclosed brassiere sizes was only slight (0.1479 ± 0.0605) using a simple Kappa analysis. Even when a Fleiss-Cohen-weighted comparison was used, only moderate agreement (0.6231 ± 0.0589) was found. The interrater agreement intraclass correlation coefficient was 0.705. Rater accuracy varied. The percentage of time spent in cosmetic practice and gender were not significantly correlated with accuracy.
Conclusions
Agreement between cup size disclosed by subjects and estimates by plastic surgeons was low. A miscommunication between the surgeon and patient may occur when using brassiere sizes to communicate wishes and estimates in procedures that involve changes in breast volume.","affiliation":"From the Department of Plastic Surgery, University of Oslo, Klinikk for Hode, Hals og Rekonstruktiv Kirurgi Oslo Universitetssykehus HF, Oslo, Norway.","publicationStatus":"epublish","language":"eng","pubModel":"Electronic-eCollection","pubTypeList":{"pubType":["research-article","Journal Article"]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1097/GOX.0000000000005046"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC10256382"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC10256382?pdf=render"}]},"commentCorrectionList":{"commentCorrection":[{"source":"MED","id":"38166302","reference":"Plast Aesthet Nurs (Phila). 2024 Jan-Mar 01;44(1):9-11. doi: 10.1097/PSN.0000000000000533","type":"Comment in","orderIn":1}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":1,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"N","license":"cc by-nc-nd","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"N","dateOfCreation":"2023-06-12","dateOfRevision":"2024-07-27","electronicPublicationDate":"2023-06-09","firstPublicationDate":"2023-06-09","firstIndexDate":"2023-06-12","fullTextReceivedDate":"2025-02-04","hasEvaluations":"N"},{"id":"24132029","source":"MED","pmid":"24132029","doi":"10.1109/jbhi.2013.2285308","title":"A computational tool for preoperative breast augmentation planning in aesthetic plastic surgery.","authorString":"Georgii J, Eder M, Bürger K, Klotz S, Ferstl F, Kovacs L, Westermann R.","authorList":{"author":[{"fullName":"Georgii J","lastName":"Georgii","firstName":"Joachim","initials":"J"},{"fullName":"Eder M","lastName":"Eder","firstName":"Maximilian","initials":"M"},{"fullName":"Bürger K","lastName":"Bürger","firstName":"Kai","initials":"K"},{"fullName":"Klotz S","lastName":"Klotz","firstName":"Sebastian","initials":"S"},{"fullName":"Ferstl F","lastName":"Ferstl","firstName":"Florian","initials":"F"},{"fullName":"Kovacs L","lastName":"Kovacs","firstName":"Laszlo","initials":"L"},{"fullName":"Westermann R","lastName":"Westermann","firstName":"Rüdiger","initials":"R"}]},"journalInfo":{"issue":"3","volume":"18","journalIssueId":2157641,"dateOfPublication":"2014 May","monthOfPublication":5,"yearOfPublication":2014,"journal":{"medlineAbbreviation":"IEEE J Biomed Health Inform","title":"IEEE journal of biomedical and health informatics","essn":"2168-2208","issn":"2168-2194","nlmid":"101604520","isoabbreviation":"IEEE J Biomed Health Inform"},"printPublicationDate":"2014-05-01"},"pubYear":"2014","pageInfo":"907-919","abstractText":"Breast augmentation was the most commonly performed cosmetic surgery procedure in 2011 in the United States. Although aesthetically pleasing surgical results can only be achieved if the correct breast implant is selected from a large variety of different prosthesis sizes and shapes available on the market, surgeons still rely on visual assessment and other subjective approaches for operative planning because of lacking objective evaluation tools. In this paper, we present the development of a software prototype for augmentation mammaplasty simulation solely based on 3-D surface scans, from which patient-specific finite-element models are generated in a semiautomatic process. The finite-element model is used to preoperatively simulate the expected breast shapes using physical soft-tissue mechanics. Our approach uses a novel mechanism based on so-called displacement templates, which, for a specific implant shape and position, describe the respective internal body forces. Due to a highly efficient numerical solver we can provide immediate visual feedback of the simulation results, and thus, the software prototype can be integrated smoothly into the medical workflow. The clinical value of the developed 3-D computational tool for aesthetic breast augmentation surgery planning is demonstrated in patient-specific use cases.","publicationStatus":"ppublish","language":"eng","pubModel":"Print-Electronic","pubTypeList":{"pubType":["Research Support, Non-U.S. Gov't","Journal Article"]},"grantsList":{"grant":[{"agency":"Federal Ministry of Economics and Technology (BMWi)","orderIn":0}]},"meshHeadingList":{"meshHeading":[{"majorTopic_YN":"N","descriptorName":"Breast","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"AH","qualifierName":"anatomy & histology"},{"majorTopic_YN":"N","abbreviation":"SU","qualifierName":"surgery"}]}},{"majorTopic_YN":"N","descriptorName":"Humans"},{"majorTopic_YN":"N","descriptorName":"Imaging, Three-Dimensional","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"Y","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Mammaplasty","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"Y","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Esthetics"},{"majorTopic_YN":"N","descriptorName":"Surgery, Plastic","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"Y","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Finite Element Analysis"},{"majorTopic_YN":"Y","descriptorName":"Computer Simulation"},{"majorTopic_YN":"N","descriptorName":"Adult"},{"majorTopic_YN":"N","descriptorName":"Female"},{"majorTopic_YN":"N","descriptorName":"Young Adult"},{"majorTopic_YN":"N","descriptorName":"Preoperative Period"}]},"subsetList":{"subset":[{"code":"IM","name":"Index Medicus"}]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1109/JBHI.2013.2285308"}]},"isOpenAccess":"N","inEPMC":"N","inPMC":"N","citedByCount":20,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"N","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"N","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"N","dateOfCompletion":"2015-02-02","dateOfCreation":"2013-10-18","dateOfRevision":"2022-03-30","electronicPublicationDate":"2013-10-10","firstPublicationDate":"2013-10-10","firstIndexDate":"2013-10-22","hasEvaluations":"N"},{"id":"22006995","source":"MED","pmid":"22006995","doi":"10.1177/1090820x11423916","title":"Four-dimensional breast imaging, part I: introduction of a technology-driven, evidence-based approach to breast augmentation planning.","authorString":"Creasman CN, Mordaunt D, Liolios T, Chiu C, Gabriel A, Maxwell GP.","authorList":{"author":[{"fullName":"Creasman CN","lastName":"Creasman","firstName":"Craig N","initials":"CN","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"drc@creasman.com"}]}},{"fullName":"Mordaunt D","lastName":"Mordaunt","firstName":"David","initials":"D"},{"fullName":"Liolios T","lastName":"Liolios","firstName":"Tom","initials":"T"},{"fullName":"Chiu C","lastName":"Chiu","firstName":"Catherine","initials":"C"},{"fullName":"Gabriel A","lastName":"Gabriel","firstName":"Allen","initials":"A"},{"fullName":"Maxwell GP","lastName":"Maxwell","firstName":"G Patrick","initials":"GP"}]},"dataLinksTagsList":{"dataLinkstag":["altmetrics"]},"journalInfo":{"issue":"8","volume":"31","journalIssueId":1879035,"dateOfPublication":"2011 Nov","monthOfPublication":11,"yearOfPublication":2011,"journal":{"medlineAbbreviation":"Aesthet Surg J","title":"Aesthetic surgery journal","essn":"1527-330X","issn":"1090-820X","nlmid":"9707469","isoabbreviation":"Aesthet Surg J"},"printPublicationDate":"2011-11-01"},"pubYear":"2011","pageInfo":"914-924","abstractText":"Although there are many three-dimensional imaging systems currently available on the market, all of them require a high degree of interaction on the part of the user, making them clinically impractical. In this article, the authors describe the development and evaluate the efficacy of a novel four-dimensional (automated three-dimensional) breast imaging system, validating it as an evidence-based simulation tool for patient consultation, surgical planning, and outcomes analysis in cosmetic breast augmentation. At six months postoperatively, simulated measurements were compared to actual measurements and questionnaires were mailed to patients asking them to assess the imaging experience. A follow-up phone survey 18 months postoperatively examined the persistence of patient attitudes about implant size and imaging. Practice productivity was evaluated by comparing specific parameters (such as scheduling rates) between three separate time periods for the same clinic.
Results
Across all parameters, breast augmentation simulations correlated highly with positive surgical outcomes (R-value = 0.68). The majority (95%) of patients believed the simulations were accurate; 89% also expressed that it enhanced trust in the surgeon and 74% reporting that it helped in choosing an implant size. Despite 48% also indicating that they would select a larger implant if they were to undergo surgery again, no patients have undergone reoperations of any kind. Compared to historical controls, scheduling rates in the practice increased from 40% to 77% after addition of simulation software.
Conclusions
4D breast imaging appears to be an accurate system for analysis, planning, simulation, and patient education for women considering primary breast augmentation, and application of this technology during the consultation process was correlated with a high degree of patient satisfaction and practice productivity.","affiliation":"drc@creasman.com","publicationStatus":"ppublish","language":"eng","pubModel":"Print","pubTypeList":{"pubType":["Research Support, Non-U.S. Gov't","Validation Study","Journal Article"]},"meshHeadingList":{"meshHeading":[{"majorTopic_YN":"N","descriptorName":"Humans"},{"majorTopic_YN":"N","descriptorName":"Diagnostic Imaging","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Breast Implantation","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"Y","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Follow-Up Studies"},{"majorTopic_YN":"N","descriptorName":"Breast Implants"},{"majorTopic_YN":"Y","descriptorName":"Computer Simulation"},{"majorTopic_YN":"Y","descriptorName":"Decision Making, Computer-Assisted"},{"majorTopic_YN":"N","descriptorName":"Adult"},{"majorTopic_YN":"N","descriptorName":"Patient Satisfaction"},{"majorTopic_YN":"N","descriptorName":"Female"},{"majorTopic_YN":"N","descriptorName":"Patient Education as Topic","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"Y","abbreviation":"MT","qualifierName":"methods"}]}}]},"subsetList":{"subset":[{"code":"IM","name":"Index Medicus"}]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1177/1090820X11424147"}]},"commentCorrectionList":{"commentCorrection":[{"source":"MED","id":"22065883","reference":"Aesthet Surg J. 2011 Nov;31(8):939-40. doi: 10.1177/1090820X11424148","type":"Comment in","orderIn":1}]},"isOpenAccess":"N","inEPMC":"N","inPMC":"N","citedByCount":33,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"N","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"N","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"N","dateOfCompletion":"2012-02-24","dateOfCreation":"2011-11-09","dateOfRevision":"2019-12-10","firstPublicationDate":"2011-11-01","firstIndexDate":"2011-11-09","hasEvaluations":"N"},{"id":"21632915","source":"MED","pmid":"21632915","doi":"10.1177/1090820x11410868","title":"Incidence of breast and chest wall asymmetries: 4D photography.","authorString":"Gabriel A, Fritzsche S, Creasman C, Baqai W, Mordaunt D, Maxwell GP.","authorList":{"author":[{"fullName":"Gabriel A","lastName":"Gabriel","firstName":"Allen","initials":"A","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Department of Plastic Surgery, Loma Linda University Medical Center, Loma Linda, California, USA. gabrielallen@yahoo.com"}]}},{"fullName":"Fritzsche S","lastName":"Fritzsche","firstName":"Sharon","initials":"S"},{"fullName":"Creasman C","lastName":"Creasman","firstName":"Craig","initials":"C"},{"fullName":"Baqai W","lastName":"Baqai","firstName":"Waheed","initials":"W"},{"fullName":"Mordaunt D","lastName":"Mordaunt","firstName":"David","initials":"D"},{"fullName":"Maxwell GP","lastName":"Maxwell","firstName":"G Patrick","initials":"GP"}]},"journalInfo":{"issue":"5","volume":"31","journalIssueId":1844051,"dateOfPublication":"2011 Jul","monthOfPublication":7,"yearOfPublication":2011,"journal":{"medlineAbbreviation":"Aesthet Surg J","title":"Aesthetic surgery journal","essn":"1527-330X","issn":"1090-820X","nlmid":"9707469","isoabbreviation":"Aesthet Surg J"},"printPublicationDate":"2011-07-01"},"pubYear":"2011","pageInfo":"506-510","abstractText":"
Background
Few authors have addressed preoperative soft tissue and chest wall analysis as it pertains to asymmetries that must be identified for preoperative breast augmentation planning.
Objectives
The authors evaluate the incidence of breast and chest wall asymmetries.
Methods
In the present study, 125 consecutive patients underwent a voluntary dimensional analysis by a single practitioner, followed by a confirmatory 4D photographic analysis. During each patient's evaluation, the following measurements were recorded: distance from nipple to inframammary fold (IMF), base width, distance from sternal notch to nipple, horizontal areolar width, vertical areolar height, upper and lower pole pinch tests, and medial and lateral pinch tests.
Results
Following exclusion of patients who had undergone prior breast surgery, 117 patients were included in the final statistical analysis. Significant differences between right and left breasts were found in 81.7% of patients in one or more of the measured dimensions (p < .05). The manual measurements were confirmed with computerized 4D photography, and there was no significant difference found between the two measurement types in any objective parameter. However, there was a significant difference in the level of chest wall asymmetries identified by 4D photography. Nipple-to-IMF position asymmetry was present in 59.6% of the patients, and sternal notch-to-nipple asymmetry was present in 81.2%. Overall, 100% of the women had some degree of asymmetry (soft tissue and/or chest wall) confirmed by 4D photography.
Conclusions
The 4D photography measurements in this study were consistent with objective manual measurements but provided the added benefit of identifying chest wall asymmetries more objectively. The data from this study underscore the importance of developing a systematic preoperative breast and chest wall analysis that can be individualized for each patient. The resulting asymmetries should then be discussed with the patient, along with the potential for continued or more pronounced asymmetry postoperatively.","affiliation":"Department of Plastic Surgery, stiffness interrogation is fundamental in breast cancer diagnosis and treatment. Furthermore, biomechanical models for predicting breast deformations have been created for several breast cancer applications. Within these applications, constitutive mechanical properties must be defined and the accuracy of this estimation directly impacts the overall performance of the model. In this study, we present an image-derived computational framework to obtain quantitative, patient specific stiffness properties for application in image-guided breast cancer surgery and interventions. The method uses two MR acquisitions of the breast in different supine gravity-loaded configurations to fit mechanical properties to a biomechanical breast model. A reproducibility assessment of the method was performed in a test-retest study using healthy volunteers and was further characterized in simulation. In five human data sets, the within subject coefficient of variation ranged from 10.7% to 27% and the intraclass correlation coefficient ranged from 0.91-0.944 for assessment of fibroglandular and adipose tissue stiffness. In simulation, fibroglandular content and deformation magnitude were shown to have significant effects on the shape and convexity of the objective function defined by image similarity. These observations provide an important step forward in characterizing the use of nonrigid image registration methodologies in conjunction with biomechanical models to estimate tissue stiffness. In addition, the results suggest that stiffness estimation methods using gravity-induced excitation can reliably and feasibly be implemented in breast cancer surgery/intervention workflows.","affiliation":"Department of Biomedical Engineering, Vanderbilt University, 5824 Stevenson Center, Nashville, TN 37235, United States of America.","publicationStatus":"ppublish","language":"eng","pubModel":"Print-Electronic","pubTypeList":{"pubType":["research-article","Journal Article"]},"grantsList":{"grant":[{"agency":"National Institute of Biomedical Imaging and Bioengineering","grantId":"R21EB022380","orderIn":0},{"agency":"NCI NIH HHS","acronym":"CA","grantId":"K25 CA204599","orderIn":0},{"agency":"NIBIB NIH HHS","acronym":"EB","grantId":"R21 EB022380","orderIn":0},{"agency":"National Cancer Institute","grantId":"K25CA204599","orderIn":0}]},"meshHeadingList":{"meshHeading":[{"majorTopic_YN":"N","descriptorName":"Breast","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"DG","qualifierName":"diagnostic imaging"},{"majorTopic_YN":"Y","abbreviation":"PA","qualifierName":"pathology"},{"majorTopic_YN":"Y","abbreviation":"SU","qualifierName":"surgery"}]}},{"majorTopic_YN":"N","descriptorName":"Humans"},{"majorTopic_YN":"N","descriptorName":"Breast Neoplasms","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"DG","qualifierName":"diagnostic imaging"},{"majorTopic_YN":"N","abbreviation":"PA","qualifierName":"pathology"},{"majorTopic_YN":"N","abbreviation":"SU","qualifierName":"surgery"}]}},{"majorTopic_YN":"N","descriptorName":"Magnetic Resonance Imaging"},{"majorTopic_YN":"Y","descriptorName":"Surgery, Computer-Assisted"},{"majorTopic_YN":"N","descriptorName":"Reproducibility of Results"},{"majorTopic_YN":"Y","descriptorName":"Gravitation"},{"majorTopic_YN":"N","descriptorName":"Female"}]},"subsetList":{"subset":[{"code":"IM","name":"Index Medicus"}]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1088/1361-6560/aa700a"},{"availability":"Free","availabilityCode":"F","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC5757162"},{"availability":"Free","availabilityCode":"F","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC5757162?pdf=render"},{"availability":"Free","availabilityCode":"F","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1088/1361-6560/aa700a"}]},"isOpenAccess":"N","inEPMC":"Y","inPMC":"Y","citedByCount":18,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"Y","authMan":"Y","epmcAuthMan":"N","nihAuthMan":"Y","manuscriptId":"NIHMS922970","hasTMAccessionNumbers":"Y","tmAccessionTypeList":{"accessionType":["doi"]},"dateOfCompletion":"2017-12-11","dateOfCreation":"2017-05-19","dateOfRevision":"2026-06-27","electronicPublicationDate":"2017-05-18","firstPublicationDate":"2017-05-18","firstIndexDate":"2017-05-20","fullTextReceivedDate":"2025-02-05","hasEvaluations":"N"}]}}
