We describe an end-to-end digital-to-surgical workflow framework for patient-specific chest wall resection and reconstruction using three-dimensional (3D) printed titanium implants. The four-stage workflow integrates Computed Tomography (CT) imaging data, virtual 3D anatomical modelling, virtual tumour-margin planning, computer-aided implant design (CAD), stereolithographic prototype validation, and additive manufacturing using laser-powder bed fusion (L-PBF). High-resolution CT data in Digital Imaging and Communications in Medicine (DICOM) format were converted into patient-specific 3D-rendered models of the chest wall and tumour to enable virtual delineation of tumour extent and digitally planned resection margins. These digital resection models were then used to design anatomically matched patient-specific implants. Implant designs were validated using stereolithographic prototypes before definitive manufacture in titanium alloy. The workflow underwent clinical feasibility evaluation in six consecutive patients requiring major chest wall resection and reconstruction. Across all cases, digitally planned resections and reconstructions were performed without intraoperative modification. The implants demonstrated accurate anatomical fit, secure fixation, and satisfactory restoration of chest wall anatomy. During the available follow-up period ranging from 5 months to 5 years, no implant failures or implant-related complications were observed. Two patients who underwent reconstruction for palliation subsequently died from progression of their underlying disease. This proof-of-concept single-centre study demonstrates the clinical feasibility of a digital-to-surgical workflow and provides a foundation for future multicentre evaluation.","affiliation":"Department of Cardiothoracic Surgery, Morriston Hospital, Swansea, Wales, UK. well-characterized mechanotransduction pathways. However, translating insights from simplified in vitro systems to complex in vivo responses remains a challenge. This review provides a framework for assessing the translational relevance of mechanobiological studies across different scales-from single cells and organoids to tissues and organs. First, we categorize soft biomaterials based on their architecture and mechanical features to establish consistent terminology. We then investigate how cells interpret the transmitted mechanical signals in advanced in vitro systems that simulate physiologically relevant mechanical environments. To discuss the translational potential, we present a scoring system and a comparative analysis demonstrating that physiological specificity, rather than experimental complexity, determines predictive value. Simple systems can yield highly translatable outcomes when mechanical cues mimic native conditions, while advanced models require careful validation to ensure their relevance. By integrating biomechanics, electrophysiology, and systems biology, we outline principles and validation strategies that enhance the predictive utility of mechanobiological findings and ultimately support the design of more effective biomaterials and tissue-engineered systems.","affiliation":"Institute of Biomechanics, Graz University of Technology, Graz, unprecedented control over the architecture, composition, and bioactivity of epidermal patches. These developments have broadened the scope of epidermal patches across biomedical and personal-care applications, supporting personalized and adaptive solutions for drug delivery, wound management, tissue regeneration, and skin-related interventions. This review summarizes next-generation printed epidermal patches, covering both conventional (non-microneedle) systems and microneedle-integrated platforms. Particular emphasis is placed on emerging material systems, including self-oxygenating hydrogels, nanomaterial-free bioinks derived from proteins and polysaccharides, and functional nanocomposite formulations. We examine key 3D printing strategies for fabricating acellular constructs, cell-laden matrices, and microneedle array patches (MAPs), alongside recent advances in multidimensional printing technologies. Biomedical applications are discussed with a focus on dermal and transdermal drug delivery, particularly insulin delivery for diabetes management as well as wound repair, regenerative therapies, photodynamic treatments, and biosensing. Additionally, the integration of printed epidermal patches with wearable sensors, smart devices, and artificial intelligence (AI) is highlighted as an emerging frontier in intelligent skin-interfaced systems, with implications for both healthcare and advanced personal-care technologies. Finally, key challenges related to clinical translation, regulatory pathways, and commercialization are addressed, providing strategic insights to guide the advancement of hydrogel-based additive manufacturing from laboratory innovation to real-world clinical and aesthetic applications.","affiliation":"Department of Pharmaceutics, Faculty of Pharmacy, Alexandria University, but overlooked determinant of long-term postoperative outcomes. Compromised bone quality is associated with hardware loosening, pseudoarthrosis, proximal junctional failure, and revision surgery. This narrative review aims to highlight the importance of preoperative bone optimization and propose a pragmatic clinical pathway for spine surgeons. Specifically, we aimed to: (1) outline risk assessment and diagnostic strategies, including clinical risk factors, laboratory testing, and imaging such as dual-energy x-ray absorptiometry (DEXA) and opportunistic CT-based Hounsfield Unit (HU) analysis; (2) evaluate therapeutic options, emphasizing pharmacologic agents (teriparatide, romosozumab, denosumab, and bisphosphonates) alongside non-pharmacologic measures including nutrition and lifestyle modification; and (3) explore future directions, including therapy duration and economic barriers to wider adoption of newer agents.MethodsThis narrative review synthesizes the current literature on preoperative bone health management in spine surgery. Risk assessment strategies were discussed including imaging, laboratory testing, and clinical picture analysis. Therapeutic options evaluated emphasize pharmacologic agents alongside non-pharmacologic measures.ResultsDespite the clinical relevance of compromised bone quality to poor surgical outcomes, bone health assessment remains inconsistently incorporated into surgical planning. Current evidence supports a multimodal approach combining targeted assessment and timely intervention to mitigate risk of adverse bone-related events following spine surgery.ConclusionsPreoperative bone health optimization is a crucial opportunity to improve long-term outcomes for patients undergoing spine surgery. Adoption of a standardized clinical pathway for risk stratification, diagnosis, and treatment may provide a valuable framework for improving fixation stability and fusion rates.","affiliation":"Rothman Orthopaedic Institute, Thomas Jefferson University Hospital, Philadelphia, PA, USA.","publicationStatus":"ppublish","language":"eng","pubModel":"Print-Electronic","pubTypeList":{"pubType":["review-article","Review","Journal Article"]},"keywordList":{"keyword":["Osteoporosis","Bone Health","Spine Surgery","Preoperative Optimization"]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1177/21925682261431791"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC12965892"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC12965892?pdf=render"}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":0,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"Y","license":"cc by-nc-nd","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"Y","tmAccessionTypeList":{"accessionType":["doi"]},"dateOfCompletion":"2026-09-28","dateOfCreation":"2026-03-06","dateOfRevision":"2026-09-28","electronicPublicationDate":"2026-03-06","firstPublicationDate":"2026-03-06","firstIndexDate":"2026-03-07","fullTextReceivedDate":"2026-03-08","hasEvaluations":"N"},{"id":"42644935","source":"MED","pmid":"42644935","pmcid":"PMC13512652","fullTextIdList":{"fullTextId":["PMC13512652"]},"doi":"10.3390/gels12080691","title":"Advances in 3D Bioprinting for Scaffold-Based and Scaffold-Free Tissue Engineering and Regenerative Medicine.","authorString":"Narayanan KB.","authorList":{"author":[{"fullName":"Narayanan KB","lastName":"Narayanan","firstName":"Kannan Badri","initials":"KB","authorId":{"type":"ORCID","value":"0000-0003-2789-5556"},"authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"School of Chemical Engineering, Yeungnam University, 280 Daehak-Ro, Gyeongsan 38541, Gyeongbuk, Republic of Korea."},{"affiliation":"Research Institute of Cell Culture, Yeungnam University, 280 Daehak-Ro, Gyeongsan 38541, Gyeongbuk, Republic of Korea."}]}}]},"authorIdList":{"authorId":[{"type":"ORCID","value":"0000-0003-2789-5556"}]},"dataLinksTagsList":{"dataLinkstag":["supporting_data"]},"journalInfo":{"issue":"8","volume":"12","journalIssueId":4246078,"dateOfPublication":"2026 Aug","monthOfPublication":8,"yearOfPublication":2026,"journal":{"medlineAbbreviation":"Gels","title":"Gels (Basel, Switzerland)","essn":"2310-2861","issn":"2310-2861","nlmid":"101696925","isoabbreviation":"Gels"},"printPublicationDate":"2026-08-01"},"pubYear":"2026","pageInfo":"691","abstractText":"Three-dimensional (3D) bioprinting has emerged as a versatile biofabrication strategy that enables the precise, spatiotemporally controlled co-deposition of living cells, biomaterials, and bioactive agents, including growth factors, cytokines, and extracellular matrix (ECM) components, into geometrically defined 3D constructs. By translating digital design models derived from computed tomography (CT), magnetic resonance imaging (MRI), or computational modeling directly into physical tissue architectures, 3D bioprinting facilitates the assembly of hierarchically organized constructs that closely recapitulate the structural, mechanical, and functional characteristics of native tissues. The principal 3D bioprinting strategies are broadly classified into scaffold-based and scaffold-free approaches. Engineered bioinks, whether formulated as cell-laden natural, synthetic, or composite polymer hydrogels, tissue-derived decellularized ECM (dECM) components, or pure cellular spheroids and organoids, constitute the cornerstone of these biofabrication platforms. Scaffold-based 3D bioprinting comprises extrusion-based, droplet-based (inkjet and drop-on-demand), light-based vat photopolymerization (stereolithography and digital light processing), and laser-assisted bioprinting based on laser-induced forward transfer (LIFT). Each of these modalities imposes distinct constraints on bioink rheology, crosslinking mechanisms, spatial resolution, throughput, and post-printing cell viability; consequently, a specific 3D bioprinting strategy is selected according to the specific requirements of the target tissue application. Scaffold-free 3D bioprinting and bioassembly techniques, including the Kenzan method, aspiration-assisted bioprinting, magnetic bioprinting, and other field-directed tissue assembly approaches, enable the fabrication of spheroid- and organoid-based constructs without the necessity for exogenous biomaterial scaffolds. Because native tissues exhibit diversity in cellular composition, ECM architecture, mechanical properties, and physiological function, no individual bioprinting platform or bioink formulation serves as a universal 3D bioprinting solution. The engineering of biomimetic tissue constructs, therefore, requires the selection of application-tailored fabrication approaches. Under this biofabrication paradigm, 3D bioprinting has been applied across a wide range of tissue engineering targets, including skin, bone, cartilage, osteochondral interfaces, cardiac and vascular tissue, neural structures, ocular, dental, and adipose tissue. This review discusses recent advances in scaffold-based and scaffold-free 3D bioprinting applications for tissue engineering and regenerative medicine across diverse tissue systems.","affiliation":"School of Chemical Engineering, Yeungnam University, 280 Daehak-Ro, Gyeongsan 38541, Gyeongbuk, Republic of Korea.","publicationStatus":"epublish","language":"eng","pubModel":"Electronic","pubTypeList":{"pubType":["review-article","Review","Journal Article"]},"grantsList":{"grant":[{"agency":"National Research Foundation of Korea","grantId":"RS-2023-00278268, RS-2025-00554349, and 2020R1A6A1A03044512","orderIn":0}]},"keywordList":{"keyword":["Tissue engineering","Regenerative Medicine","3D Printing","Droplet-based","Scaffold-free","Bioassembly","Extrusion-based","Scaffold-Based","Light-based","Laser-mediated"]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.3390/gels12080691"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13512652"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13512652?pdf=render"}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":0,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"Y","license":"cc by","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"Y","tmAccessionTypeList":{"accessionType":["doi"]},"dateOfCompletion":"2026-08-26","dateOfCreation":"2026-08-26","dateOfRevision":"2026-08-28","electronicPublicationDate":"2026-08-03","firstPublicationDate":"2026-08-03","firstIndexDate":"2026-08-27","fullTextReceivedDate":"2026-09-10","hasEvaluations":"N"},{"id":"41740294","source":"MED","pmid":"41740294","pmcid":"PMC13263067","fullTextIdList":{"fullTextId":["PMC13263067"]},"doi":"10.1016/j.biomaterials.2026.124087","title":"Multimodal characterization of sustained bioagent release from an epicardial depot for long-term biomaterial incorporation.","authorString":"Varela CE, Monahan DS, Fan Y, Islam S, Kelleher JT, Whyte W, Bonnemain J, Ngoy S, Fisch S, Wallace E, Ronan W, Beatty R, Dolan EB, Nguyen CT, Duffy GP, Roche ET.","authorList":{"author":[{"fullName":"Varela CE","lastName":"Varela","firstName":"Claudia E","initials":"CE","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Institute for Medical Engineering and Science, Massachusetts Institute of Technology, Cambridge, MA, USA; Replenishable reservoir systems have enabled localized bioagent delivery to the epicardial surface but therapy transport from these systems is constrained by semipermeable membranes and fibrous capsule formation. Our approach to improved therapy delivery from epicardial reservoir systems is multi-pronged. First, we introduce a membrane-free reservoir system by incorporating a gelatin scaffold into a flexible polymer implant to promote direct integration with the epicardial surface and act as a replenishable depot to encourage myocardial-directed transport. Next, we perform in vitro and ex vivo validations and multi-scale computational simulations to characterize biomaterial, tissue, and organ-level transport of therapy, considering both native tissue architecture, and the effect of blood vessel clearance. As an in vivo use case of our system, we investigated the functional effect of multi-dose regimens of human follistatin-like 1 protein (FSTL1) in a rat model of myocardial infarction (MI). Groups receiving multiple doses of FSTL1 show increased cardiac performance (ejection fraction and fractional shortening), and decreased chamber stiffness 28 days after MI. Multi-dosing increases ventricular wall thickness and reduces infarct size. We demonstrate a dose-dependent increase in blood vessel number and density in the infarct zone. Finally, we establish a computational and experimental framework for patient-specific modeling to optimize implant parameters such as reservoir size and shape, infarct location, and dosing regimens, with a vision for clinical-imaging guided bioagent delivery strategies that can be modified on a per-patient, therapy-specific basis to optimize dosing regimens of various bioagents. This study highlights the potential for integrating personalized computational models with replenishable delivery systems to improve bioagent transport from biomaterials and enhance post-MI therapeutic outcomes.","affiliation":"Institute for Medical Engineering and Science, Massachusetts Institute of Technology, Cambridge, MA, USA; Harvard-MIT Program in Health Sciences and Technology, evaluation, and regulatory approval of medical devices is expanding rapidly. These approaches offer clear benefits in accelerating innovation, reducing development burden, and strengthening safety assessment, yet their regulatory acceptance remains inconsistent across jurisdictions. This variability reflects differences in model maturity, documentation practices, and the absence of globally harmonised criteria for assessing credibility. Building on established standards and regulatory guidance, this position paper outlines a risk-informed framework for the credible generation, assessment, and regulatory utilisation of in silico evidence that could be implemented across the medical device lifecycle. The framework emphasises clear definition of a question of interest and context of use, proportional credibility requirements based on model risk, and rigorous application of verification, validation, and uncertainty quantification. Emerging challenges posed by data-driven and hybrid models, including artificial intelligence, highlighting additional governance and data integrity considerations are also addressed. Importantly, the generation of in silico evidence increasingly relies on bioinformatics and computational biomedicine workflows, including the integration of heterogeneous biomedical data sources (e.g. medical imaging, real-world data, and population-level datasets), preprocessing and feature extraction pipelines, and hybrid mechanistic-data-driven modelling approaches. In this perspective, the authors argue that the credibility of in silico evidence is not determined solely at the level of the executable model, but across the full computational pipeline, encompassing data provenance, preprocessing, model implementation, and reproducible analysis workflows.","affiliation":"Synopsys, Inc., 1007 Church Street, Evanston, IL 60201-5912, United implant (CI) placement is difficult. Although ZIs show favourable outcomes, factors influencing their survival remain unclear. This retrospective study evaluated implant- and patient-related factors associated with ZI survival in full-arch immediate-loading rehabilitation using the all-on-four concept combining ZIs and CIs.
Methods
A total of 923 implants (323 ZIs and 600 CIs) placed in 203 patients between 2010 and 2021 were analysed. Cumulative survival rates were estimated using the Kaplan-Meier method, and intergroup comparisons were performed using the log-rank test. Cox proportional hazards regression analyses were performed to calculate hazard ratios (HRs). Statistical significance was set at p < 0.05.
Results
Cumulative survival rates at 3-13 years were 94.5% and 95.9% at the patient and implant levels, respectively, for ZIs, and 97.9% and 98.7%, respectively, for CIs. ZIs showed significantly lower implant-level survival than CIs (p = 0.0178). Palatal positioning of the ZI platform was significantly associated with reduced implant-level survival (HR = 18.177, 95% CI: 1.418-233.053, p = 0.026). Systemic disease was significantly associated with reduced patient-level ZI survival after adjustment for sex and smoking status (HR = 14.872, 95% CI: 1.812-122.077, p = 0.012).
Conclusions
Immediate full-arch rehabilitation combining ZIs and CIs achieved high long-term survival; however, palatal platform positioning and systemic disease were associated with ZI failure.","affiliation":"Department of Implant Dentistry, Showa Medical University School of Dentistry, 2-1-1, Kita-senzoku, Ota-ku, Tokyo, 145-8515, Japan. cancer screening and longitudinal monitoring require imaging technologies that are portable, operator-independent, and suitable for frequent use, a capability not fully met by mammography or conventional ultrasound. We present a three-dimensional (3D) portable ultrasound system for real-time examination (3D PURE) that overcomes key limitations in volumetric breast imaging through advances in transducer design, acoustic materials, and adaptive beamforming. A box-array design incorporating a corner-gap offset geometry suppresses peak crosstalk (by 3.73 dB at the corner-most element), prevents preamplifier saturation, and supports higher transmit voltages (up to 24 V). A custom flowable backing layer (impedance 6.12 MRayl; attenuation 7.56 dB mm-1 MHz-1) integrates around fragile wirebonds, reduces inter-element crosstalk by ~4.5 dB throughout the array, and improves axial and lateral/elevational resolutions by ~200 µm and ~70 µm, respectively. Layered Aberration-Correction Reconstruction (LACR), an adaptive 3D beamformer, compensates for heterogeneous speed-of-sound (SoS) in the breast, reducing depth localization error by 2 mm and aberration defocusing by 70 µm on average at a 5 cm depth. Nine of the ten participants in an in vitro study showed improved microtarget detection efficiency with 3D PURE relative to a conventional 2D system (p = 0.0215), analogous to detection of microcalcifications. These results, combined with in vivo imaging validation of various phenotypes, highlight the potential of 3D PURE for reliable breast imaging. Furthermore, a vision-guided computer interface, MyFUS, ensures self-guided, user-friendly, and operator-independent probe positioning for longitudinal monitoring.","affiliation":"Media Lab, Massachusetts Institute of Technology, Cambridge, MA, USA.","publicationStatus":"epublish","language":"eng","pubModel":"Electronic","pubTypeList":{"pubType":["Research Support, Non-U.S. Gov't","research-article","Journal Article"]},"meshHeadingList":{"meshHeading":[{"majorTopic_YN":"Y","descriptorName":"Breast","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"DG","qualifierName":"diagnostic imaging"}]}},{"majorTopic_YN":"N","descriptorName":"Humans"},{"majorTopic_YN":"Y","descriptorName":"Breast Neoplasms","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"DG","qualifierName":"diagnostic imaging"}]}},{"majorTopic_YN":"Y","descriptorName":"Imaging, Three-Dimensional","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"IS","qualifierName":"instrumentation"},{"majorTopic_YN":"N","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"Y","descriptorName":"Ultrasonography, Mammary","meshQualifierList":{"meshQualifier":[{"majorTopic_YN":"N","abbreviation":"IS","qualifierName":"instrumentation"},{"majorTopic_YN":"N","abbreviation":"MT","qualifierName":"methods"}]}},{"majorTopic_YN":"N","descriptorName":"Equipment Design"},{"majorTopic_YN":"N","descriptorName":"Phantoms, Imaging"},{"majorTopic_YN":"N","descriptorName":"Transducers"},{"majorTopic_YN":"N","descriptorName":"Female"}]},"subsetList":{"subset":[{"code":"IM","name":"Index Medicus"}]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.1038/s41467-026-74708-3"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13323354"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13323354?pdf=render"}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":0,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"Y","hasLabsLinks":"N","hasData":"Y","license":"cc by-nc-nd","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"N","dateOfCompletion":"2026-07-01","dateOfCreation":"2026-07-01","dateOfRevision":"2026-08-13","electronicPublicationDate":"2026-07-01","firstPublicationDate":"2026-07-01","firstIndexDate":"2026-07-02","fullTextReceivedDate":"2026-08-19","hasEvaluations":"N"},{"id":"42213103","source":"MED","pmid":"42213103","pmcid":"PMC13433372","fullTextIdList":{"fullTextId":["PMC13433372"]},"doi":"10.1007/s00266-026-05919-8","title":"Does Patient History Influence Capsular Contracture? Although numerous patient-, surgical-, and implant-related risk factors have been proposed, reliable population-level predictors remain inconsistent across studies. This study evaluates whether administrative medical history, as encoded by ICD and CPT codes, contains sufficient predictive signal to identify patients at risk for CC using machine learning.
Methods
Patients were queried from the MerativeTM MarketScan® Research Databases from 2003 to 2017 with CPT codes for implant-based breast reconstruction and augmentation. ICD codes were then used to identify all events and conditions of a patient's history. Hyperparameter-tuned random forest models were combined with multivariable logistic regression models to validate risk factors for the development of CC.
Results
A total of 112,489 patients were included, and the rate of capsular contracture was 9.55%. In total, 4,825 common conditions and procedures were included as features in the model. The random forest's error rate was 9.54%. Prior CC was the most important variable in the model. When removed, model accuracy decreased by 0.0011% (p <0.001). Removing prior irradiation as a feature decreased model accuracy by 0.00014% (p < 0.001). In line with the previous literature, prior CC (OR = 2.47, p <0.001) and irradiation (OR = 1.16, p <0.001) were significantly associated with CC development in a multivariable logistic regression model.
Conclusions
Administrative medical history alone demonstrated limited predictive utility for CC development. Although established associations such as prior CC and irradiation were confirmed, their incremental contribution to predictive performance was negligible. These associations do not meaningfully enhance a clinician's ability to reliably predict which patients are at increased risk for CC.
Level of evidence iii
This journal requires that authors assign a level of evidence to each article. robust and durable tip support is required. However, conventional full-thickness harvest is associated with significant donor-site morbidity, and commonly employed rigid fixation strategies-such as the septal extension graft-substantially restrict postoperative nasal tip compliance. The present study introduces a novel two-component technique combining a half-harvest costal cartilage procurement method with a pyramidal-shaped columellar strut graft anchored on the floating-tip principle, with the objective of maintaining postoperative nasal tip flexibility while providing structural support following augmentation rhinoplasty. Methods: A retrospective review was performed of consecutive patients who underwent primary or revision augmentation rhinoplasty using the pyramidal costal cartilage columellar strut graft technique by a single surgeon between June 2018 and February 2026. The medial half of the conjoined costal cartilage at the seventh, eighth, or ninth rib was procured via a half-harvest approach, preserving the lateral cortex and perichondrium to minimize donor-site morbidity and potential cartilage regeneration was considered a theoretical benefit. The harvested cartilage was carved into a pyramidal columellar strut and secured to the anterior nasal spine using a floating fixation construct; the inferior base of the strut was rigidly fixed to the nasal septum and anterior nasal spine with a minimum of three PDS 5-0 sutures, while the superior portion remained free to preserve physiologic nasal tip mobility. Adjunctive cap and shield grafts, perichondrial wrapping, and dermal fat grafts were employed as indicated. Primary outcomes included nasal tip projection, postoperative tip mobility, donor-site morbidity, and surgical complication rates. Results: Favorable clinical observations of maintained tip projection were noted throughout follow-up. Manual postoperative examination suggested preservation of tip flexibility in most patients; however, no validated objective mobility assessment tool was available. The revision rate for clinically significant tip deviation was low. No major donor-site adverse events-including pneumothorax or rib fracture-were encountered. Postoperative chest wall pain was minimal and transient, with most patients resuming daily activities within one week of surgery. Conclusions: The pyramidal-shaped costal cartilage columellar strut graft with half-harvest technique is a novel, biomechanically informed, and technically reproducible approach to augmentation rhinoplasty that was developed to address donor-site morbidity and postoperative tip rigidity, two commonly recognized limitations of conventional costal cartilage rhinoplasty: donor-site morbidity and postoperative nasal tip rigidity. Preservation of the lateral cortex and perichondrium during procurement may contribute to reduced postoperative donor-site discomfort, accelerates functional recovery, and may promote endogenous cartilage regeneration over time. The anatomically derived pyramidal strut geometry, combined with floating fixation to the anterior nasal spine, was designed to approximate the native columellar architecture, enabling consistent preservation of physiologic nasal tip mobility. The present series demonstrated a favorable safety profile with a low overall complication rate and an absence of major donor-site adverse events. Prospective studies with validated objective outcome measures are required to confirm these findings, to delineate the optimal patient selection criteria, and to establish evidence-based long-term outcome benchmarks for this technique.","affiliation":"Department of Plastic and Reconstructive Surgery, Gachon University Gil Medical Center, Incheon 21565, Republic of Korea.","publicationStatus":"epublish","language":"eng","pubModel":"Electronic","pubTypeList":{"pubType":["research-article","Journal Article"]},"keywordList":{"keyword":["Augmentation Rhinoplasty","Costal cartilage","Columellar Strut Graft","Floating Tip","Half-harvest Technique","Nasal Tip Mobility","Pyramidal Graft","Rib Cartilage Rhinoplasty"]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.3390/jcm15134985"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13362741"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13362741?pdf=render"}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":1,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"Y","hasLabsLinks":"N","hasData":"Y","license":"cc by","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"N","dateOfCompletion":"2026-07-15","dateOfCreation":"2026-07-15","dateOfRevision":"2026-08-13","electronicPublicationDate":"2026-06-26","firstPublicationDate":"2026-06-26","firstIndexDate":"2026-07-16","fullTextReceivedDate":"2026-09-03","hasEvaluations":"N"},{"id":"42332744","source":"MED","pmid":"42332744","pmcid":"PMC13539735","fullTextIdList":{"fullTextId":["PMC13539735"]},"doi":"10.1186/s12951-026-04655-0","title":"Decoding triple negative breast cancer bone metastasis: from 3D bioprinted models to clinical translation.","authorString":"Xu J, Kang Y, Song K, Fan J, Liu Y.","authorList":{"author":[{"fullName":"Xu J","lastName":"Xu","firstName":"Jiahong","initials":"J","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Department of Pancreatic Diagnosis and Treatment Center, Cancer Hospital of Dalian University of Technology, Liaoning Cancer Hospital and Institute, Shenyang, China."}]}},{"fullName":"Kang Y","lastName":"Kang","firstName":"Yue","initials":"Y","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Department of Breast Cancer Surgery, Cancer Hospital of Dalian University of Technology, Liaoning Cancer Hospital and Institute, Shenyang, China."}]}},{"fullName":"Song K","lastName":"Song","firstName":"Kedong","initials":"K","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Department of Pancreatic Diagnosis and Treatment Center, Cancer Hospital of Dalian University of Technology, Liaoning Cancer Hospital and Institute, Shenyang, China. kedongsong@dlut.edu.cn."},{"affiliation":"State Key Laboratory of Fine Chemicals, Dalian University of Technology, 2 Linggong Road, Dalian, 116024, China. kedongsong@dlut.edu.cn."}]}},{"fullName":"Fan J","lastName":"Fan","firstName":"Jiangli","initials":"J","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"State Key Laboratory of Fine Chemicals, Dalian University of Technology, 2 Linggong Road, Dalian, 116024, China. fanjl@dlut.edu.cn."}]}},{"fullName":"Liu Y","lastName":"Liu","firstName":"Yefu","initials":"Y","authorAffiliationDetailsList":{"authorAffiliation":[{"affiliation":"Department of Pancreatic Diagnosis and Treatment Center, Cancer Hospital of Dalian University of Technology, Liaoning Cancer Hospital and Institute, Shenyang, China. 97902153@cmu.edu.cn."}]}}]},"dataLinksTagsList":{"dataLinkstag":["supporting_data"]},"journalInfo":{"issue":"1","volume":"24","journalIssueId":4213834,"dateOfPublication":"2026 Jun","monthOfPublication":6,"yearOfPublication":2026,"journal":{"medlineAbbreviation":"J Nanobiotechnology","title":"Journal of nanobiotechnology","essn":"1477-3155","issn":"1477-3155","nlmid":"101152208","isoabbreviation":"J Nanobiotechnology"},"printPublicationDate":"2026-06-01"},"pubYear":"2026","pageInfo":"830","abstractText":"This review decodes the biological basis of triple-negative breast cancer (TNBC) bone metastasis and critically examines how 3D bioprinted models can bridge mechanistic investigation and clinical translation. We first synthesize the osteolytic vicious cycle, multicellular microenvironmental crosstalk, and pre-metastatic niche remodeling that drive TNBC bone colonization, and then evaluate how bioprinting-based platforms can recapitulate these processes with greater spatial, mechanical, and cellular fidelity than conventional models. Traditional static research models fail to capture this dynamic spatiotemporal complexity, hindering understanding and treatment development. 3D bioprinting overcomes this by creating a biomimetic microenvironment. It uses hydrogel or composite scaffolds to replicate the bone matrix's mechanical properties and biochemical gradients. This integrates tumor cells, vascular networks, and bone stromal cells, dynamically modeling metastatic colonization. This bioprinted system serves as a high-efficiency drug screening platform, enabling systematic assessment of nano-drug delivery and strategies to overcome microenvironment-induced resistance. Clinically, 3D printing directly aids in developing patient-specific bone repair implants and surgical guides for reconstructing metastatic lesions. Critically, these models bridge fundamental research and clinical translation. They allow spatiotemporal visualization/analysis of key signaling pathways in metastasis and facilitate high-throughput screening of bone-targeted combination therapies. Rather than presenting a newly established experimental model, this article provides a focused review of current advances, unresolved technical barriers, and future translational directions for TNBC-oriented 3D bioprinting in bone-metastasis research.","affiliation":"Department of Pancreatic Diagnosis and Treatment Center, Cancer Hospital of Dalian University of Technology, Liaoning Cancer that estimates breast hyperelastic parameters from in vivo, gravity-driven deformation, and to investigate whether post-augmentation breasts with silicone implants exhibit altered mechanical characteristics compared with natural breasts.
Methods
Left breasts of twelve participants (six normal, six post-augmentation) were modeled from prone MRI, and upright morphology was simulated using a two-step gravity procedure in Abaqus using a first-order isotropic Ogden hyperelastic law, chosen as a simple, widely used model for large soft-tissue deformations. For each breast, the Ogden exponent (α) and shear modulus (μ) were identified as apparent organ-level stiffness parameters by minimizing the Hausdorff distance between the simulated shape and the standing 3D surface scan.
Results
Natural breasts achieved HDF 0.94-3.09 mm (mean 2.14 mm). Post-augmentation breasts achieved HDF 1.38-2.54 mm (mean 1.90 mm). Across participants, α showed marked inter-individual variability. Post-augmentation cases required higher effective shear modulus than natural breasts.
Conclusion
A subject-specific FE workflow driven by in vivo deformation accurately reproduces standing breast morphology and reveals increased apparent stiffness after implant augmentation. This framework enables individualized parameterization for surgical planning, implant selection, and prediction of post-operative outcomes in breast biomechanics.","affiliation":"Department of Plastic of patient-specific biomedical implants, with growing importance in advanced drug delivery systems. Beyond structural tissue engineering, bioprinted constructs now function as programmable therapeutic depots capable of localized, sustained, and stimuli-responsive drug release. This review focuses on recent biomaterial design strategies that enable precise control over drug encapsulation, retention, and release kinetics within 3D bioprinted architectures. The physicochemical and mechanical properties of bioinks, including crosslinking density, porosity, degradation behavior, viscoelasticity, and swelling characteristics, directly influence drug loading efficiency and release dynamics under physiological conditions. The rational tuning of these parameters allows the development of constructs that provide spatially controlled and temporally regulated therapeutic delivery. Recent advances in predictive modeling, such as finite element modeling (FEM), data-driven machine learning approaches, and ML, have significantly improved the ability to correlate material composition, printing parameters, and structural geometry with drug diffusion and degradation-mediated release mechanisms. These tools facilitate the optimization of printing variables including extrusion pressure, nozzle diameter, and layer resolution to ensure structural fidelity while maintaining therapeutic functionality. Emerging strategies incorporating multi-material printing, gradient architectures, and stimuli-responsive biomaterials have expanded the potential of 3D bioprinting for combination therapies and personalized medicine. This review discusses key challenges in translating bioprinted drug delivery systems into clinical applications, including the standardization of drug release characterization methods, and long-term stability assessment.","affiliation":"Industry 4.0 Convergence Bionics Engineering, Department of Biomedical Engineering, Pukyong National University, Busan 48513, Republic of Korea.","publicationStatus":"epublish","language":"eng","pubModel":"Electronic","pubTypeList":{"pubType":["review-article","Review","Journal Article"]},"grantsList":{"grant":[{"agency":"Pukyong National University","grantId":"202412240001","orderIn":0},{"agency":"National Research Foundation of Korea","grantId":"2022R1A5A8023404","orderIn":0}]},"keywordList":{"keyword":["Tissue engineering","Biomaterials","Scaffolds","Computational Modelling","Mechanical Characterization","3D Printing"]},"fullTextUrlList":{"fullTextUrl":[{"availability":"Subscription required","availabilityCode":"S","documentStyle":"doi","site":"DOI","url":"https://doi.org/10.3390/ma19112186"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"html","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13257692"},{"availability":"Open access","availabilityCode":"OA","documentStyle":"pdf","site":"Europe_PMC","url":"https://europepmc.org/articles/PMC13257692?pdf=render"}]},"isOpenAccess":"Y","inEPMC":"Y","inPMC":"Y","citedByCount":1,"hasReferences":"Y","hasTextMinedTerms":"Y","hasDbCrossReferences":"N","hasPDF":"Y","hasBook":"N","hasSuppl":"N","hasLabsLinks":"N","hasData":"Y","license":"cc by","authMan":"N","epmcAuthMan":"N","nihAuthMan":"N","hasTMAccessionNumbers":"Y","tmAccessionTypeList":{"accessionType":["doi"]},"dateOfCompletion":"2026-06-12","dateOfCreation":"2026-06-12","dateOfRevision":"2026-08-13","electronicPublicationDate":"2026-05-22","firstPublicationDate":"2026-05-22","firstIndexDate":"2026-06-12","fullTextReceivedDate":"2026-06-13","hasEvaluations":"N"}]}}
